“The funding of the MACSS Il project through the generous Rubner grant by Stiftung Charité enabled the full deployment of a highly functional telemedical support system for routine care of renal transplant recipients at the Charité – Universitätsmedizin Berlin. During the COVID pandemic we further extended telemedical services to acutely ill patients postive for the SARS-COV2 virus. This now allows to monitorvital signs of acute i ll patients and patients with chronic diseases directly at home in real time. Patients are receiving actual laboratory values and an updated medication plan, which was checked for drug-drug interactions, on a smartphone app. In addition, laboratory values are automatically exchanged between treating physicians at Charité and home nephrologists. The close monitoring of vital signs and well-being allows the timely detection of complications by a dedicated telemedicine team, who may contact the patient and decide on the next steps, e.g. whether the patient should be referred to his home doctor, nephrologist or the emergency room. The medication plan on the smartphone is compatible with the German standardized medication plan and may be adjusted by the patient. This shoud increase empowerment and safety and provides the doctor With the actual drug intake. The reminder and confirmation functionality supports and documents adherence. Decreasing adherence may lead to additional educational sessions through the telemedicine team. The receipt of lab values and medication plan actively increases selfparticipation of the patient in the treatment process. Sharing of lab values and medication plan with home nephrologists across different sectors in the German health system increases and simplifies communication and all values can be integrated into the treatment. The goal is to avoid or reduce unplanned hospitalizations, reduce costs for the health care system and increase quality of life of the patient.”
